Neuroplasticity claims about ibogaine: what preclinical models actually show

Animal studies suggest ibogaine and noribogaine upregulate GDNF and BDNF expression. Translating those findings into claims about clinical efficacy requires distinctions that popular coverage often blurs.

Rodent studies dating back to the early 2000s have documented sustained increases in glial cell line-derived neurotrophic factor (GDNF) and brain-derived neurotrophic factor (BDNF) following ibogaine administration. These findings have been replicated in independent laboratories and form the principal mechanistic case for the compound's purported anti-addictive effects.

The translational gap, however, is substantial. Preclinical evidence of neuroplasticity in rodent ventral tegmental area does not constitute evidence of clinical efficacy in human substance use disorders, and conflating the two is a recurring problem in lay coverage.

Human imaging studies remain limited and largely uncontrolled. What can be said with confidence is that the mechanistic hypothesis is plausible and worth testing; what cannot yet be said is that it has been tested adequately in the populations to which it is most often applied.
